Health Coaching Agreement

1. Services Coach agrees to provide health coaching and integrative nutrition services as described in the selected coaching package. Services may include but are not limited to consultations, customized plans, and guidance. 2. Client Responsibilities Client understands that success depends on their own commitment, effort, and consistency. Coach cannot guarantee any specific results, as outcomes are based on the Client’s personal actions, health status, and adherence to the program. 3. Scope of Practice & No Guarantees Coach is a certified Integrative Nutritionist and provides evidence-based guidance within that scope. Coach is not a licensed medical professional and does not diagnose, treat, or prescribe for medical conditions. Coaching is educational and supportive in nature. Client acknowledges that results vary and are dependent on their own participation, therefore Coach cannot guarantee specific outcomes or satisfaction. 4. Payment & Non-Refundable Policy All payments are due in full at the time of purchase unless otherwise stated in writing. All purchases are final and non-refundable. This applies regardless of Client’s level of participation or outcome. 5. Cancellation Policy If the Client needs to reschedule a session, 24 hours’ notice is required. Sessions not rescheduled within this timeframe will be forfeited and not refunded. 6. Release of Liability Client releases Coach from any claims, damages, or liabilities resulting from participation in the program. Client assumes all risks and responsibility for implementing any suggestions provided. 7. Confidentiality Coach agrees to keep all Client information confidential except as required by law. 8. Use of Testimonials and Anonymized Results Client agrees that Coach may use anonymized data, general results, and/or written or recorded testimonials provided by the Client for marketing, educational, or promotional purposes. No identifying information will be used without Client’s explicit written consent. 9. Term & Termination This Agreement begins on the date of signature and continues for the duration of the selected coaching program. Either party may terminate with written notice. No refunds will be issued upon termination. 10. Dispute Resolution Any disputes arising from this Agreement will first attempt resolution through mediation. If unresolved, disputes will be handled according to the laws of Colorado.
